So I just watched the interview with the writers, Peter Blake and Katherine Lingenfelter.
It wasn't a hallucination. House really did crash his car into Cuddy's dining room. Nobody mentioned Vicodin, brain tumors, or any other possible cause other than pent-up rage.

I'm going to delude myself that there is something going on they can't tell us about. Until I find out otherwise, anyway. Because if this is all real and there are no mitigating factors, I'm too heartbroken.
ETA that if they weren't planning for there to be something else going on here, I wonder if the reaction they're getting from fans and critics will make them retcon something in.

That's my worst fear about this, that they'll say "uh oh" and start season 8 by saying "oh no, silly us, we forgot to tell you that House was stoned on Vicodin and thought he was driving his car into Cuddy's garage! And he has a tumour in his brain! And Bobby's in the show and Pam was dreaming! And anyway, Cuddy deserved it for daring to date someone else!".
