Tonight's Episode, Part II
So I just watched the interview with the writers, Peter Blake and Katherine Lingenfelter.
It wasn't a hallucination. House really did crash his car into Cuddy's dining room. Nobody mentioned Vicodin, brain tumors, or any other possible cause other than pent-up rage.

And it's good that from Cuddy's perspective this is a line that cannot be crossed. This will have repurcussions. Good. Wilson is still trying to figure it all out, but he'd already taken a bigger step toward cutting off House's Vicodin supply and he's worn down and uncertain.
As to where to go next, it's interesting that at least some of the other players have shortened contracts for next year. This leaves open the possibility that we will have several episodes of House alone, trying to exist and work things out without not just his support system but also his figurative crutches.
I don't think it's the best, nor do I think it was the worst, but I'm still wondering what happens next, which is what a season finale should do.

Me too. As I said to Dee upstream, I don't think House has gotten away with anything here. He's ruined his relationship with Cuddy forever, and if I were Wilson I'd run the other way if I saw him coming. To be blunt, House is fucked, and I assume the beginning of next season will be The Journey of House Toward Un-fuckdom. It'll all depend on if any viewers still want to go down that road with him. I will, but from the reactions I was seeing last night, I'm not sure a lot of other fans will. On the other hand, Internet fandom really is a drop in the bucket in the so-called great scheme of things.

YES, THIS. The potential has *always* been there. As Cuddy said in "Finding Judas," House holds himself back. His leg pain has been a sign of his internalized anger since S2. I agree, as disappointing and nasty and vicious as it was, it was in line with House's character.
